I {name-1-first-name} {name-1-last-name}, hereby grant permission for my Son/Daughter, {name-2-first-name} {name-2-last-name} to travel to the United States with and under the guidance of the UPLAY CANADA BASKETBALL organization and representatives for the purpose of participating in various amateur basketball youth events on the NIKE EYBL/MadeHoops Circuit and various tournaments. The group including my child will be crossing into the United States of America for & returning back into Canada after each tournament. The Nike EYBL Circuit begins in November and concludes in July. This letter grants permission to UPLAY CANADA BASKETBALL to cross the border with my son/daughter for ALL tournaments in the USA during the current season.

My son/daughter is in possession of the proper identification to cross the border and I am available should any concerns arise in connection with this letter.
